Health office visit Log

1. Go to: Search “PSCB Custom Reports” in the top search box in the top bar OR navigate to Data and Reporting → Reports →PSCB Custom Reports.

2. Click Health Office --> Health Office Visit Log

This report provides details on the number and list of students who visited the clinic on specific date(s), including: Treatment provided, Visit reason, Visit outcome, Staff member who created the log

Note: If any changes are made to the filter criteria, you must resubmit the request to refresh the results.
